What triggers it. Triggered by hiring. Every figure above is priced at 1 account; this line is what each unit past that costs, and it compounds for the remainder of the term rather than the remainder of the month.

How to avoid it. Audit entitlements quarterly. On a three-year term, seats assigned to people who left are the most reliably wasted money in the whole accounting budget.

Paid add-on or module

What triggers it. A separately-priced module. It stacks on top of the plan rate for as long as you keep it, and on a per-unit add-on it also scales with the 1 account basis.
