What triggers it. Usage, not people, drives this. That makes it the least predictable component of a three-year Encharge commitment and the one most worth instrumenting early.

How to avoid it. Instrument your real consumption during the trial and size the plan to the ninetieth-percentile month, not the average one. Overages priced at list are always worse than the tier that would have included them.
